My Plan to Flourish

My Plan to Flourish is a person-centered planning app for a connected (and mobile) world.

For the person-served, it is a talking vision board they can carry in their pockets that helps support staff learn important things about them like their interests, likes/dislikes, and how to best work with them to optimize plan results. This creates more meaningful and effective interactions and increases client level of satisfaction with themselves – and their relationships!

It’s mobility and customizable interface provides a more engaging way keep clients involved in working toward and achieving goals between healthcare visits or check-ins.
