The app will help our ACO providers and clinical partners in Optimus Healthcare Partners, Atlantic ACO, and Healthcare Quality Partners easily find contact information for ACO providers and resources including SNFs, MSOs, Payers, and Community Resources.

Providers can create, share, and view other ACO physicians’ medical neighborhoods (i.e., preferred networks), access Cortext and our Provider Portal.
