ACO GO!

The ACO GO! app will help the members of the Atlantic Alliance clinically integrated network and its associated ACO’s to develop and define their preferred medical neighborhoods for specialists and primary care physicians. It will be a point of care resource for those clinicians and hospital staff requiring quick and easy access to that information at the point of care. It will also provide contact information for the ACO staff and care management team, as well as a listing of collaborative post-acute care facilities.
