Fixes bug with submit button in mult-isession forms

* Option to resize photo: Original or fixed smaller size
* Support having question before/after table in grid section
Improvements:
* Restrict numeric field to accept integer only or both integer/decimal
* Upload logs for response contains photo (when response has not been synced)
* Change label 'Required' to '*'
