Caring for vulnerable populations, or those in crisis, requires care teams.

There are 13 million adults in the US with serious mental illness (SMI). SMI includes disorders such as bipolar disorder, major depressive disorder, schizophrenia, and schizoaffective disorder.

Assertive Community Treatment (ACT) is a multidisciplinary, team-based model that provides intensive community-based and outreach-oriented services to people who experience the most severe and persistent serious mental illness. ACT team members collaborate on assessments, treatment planning, and day-to-day interventions.

VelloHealth designed its VelloHealth Collaboration Platform (VCP) to serve and assist these collaborative ACT team activities.
