Total Patient Care (TPC) is an application for Healthcare Professionals to facilitate a rigorous and rapid assessment of their patients considering multiple organs and systems. This application aggregates several scientifically validated risk calculators, allowing patients to be stratified regarding renal, cardiovascular, hepatic, and other functions. This tool allows to optimize the evaluation and clinical approach of the patients comorbidities through a single resource and, consequently, a better time management for the health professionals. This tool is an aid to clinical practice and does not replace the individualized evaluation by health professionals.
