You can help us fix the antiquated forms that you come across every day - at school, soccer clubs, girl scouts, day care providers, and doctors' offices. Using this app and your mobile device, simply snap a photo of the form and hit submit - that's all you need to do!

The app also contains useful information, such as sample letters to send to doctors, asking them to update their forms.
