In the Personal Health Folder you will find medical and nursing reports, notices of upcoming appointments, your vaccination record, information on the medication you take or instructions on the lifestyles that have been recommended to you in consultation.

The folder can also be your new entry point to online services such as booking an appointment at your health center or checking your situation on waiting lists.

It also offers you interesting links with self-care tips, as well as access to our search engines for health centers and on-call pharmacies.

The Personal Health Folder is the result of the digital information generated by the centers where you receive care, and the information that you consider necessary to provide, to complete your history.

The application includes a specific section on the coronavirus COVID-19.

What do I need to access My Health Folder?

- Be 16 years old or older

- You must go to your health center and make the request in person, proving your identity.

- Have an email address and a mobile phone to receive SMS.

- If you already have credentials for having previously made the request to access the Personal Health Folder website, you do not need to do it again.
