WHO CAN USE THIS APP:

Blue Cross Blue Shield of Michigan members who received an invitation to download the app as part of the Blue Cross® Coordinated Care program. You will need your Enrollee ID and an access code to sign up.

THIS APP WILL HELP INVITED MEMBERS:

· Chat with a nurse through secure messaging

· Create reminders to track medications and set appointments

· Keep an eye on health and overall well-being progress

· Get encouragement, checklists and daily tasks to meet goals

· Access personalized education to improve their health

Blue Cross Coordinated Care is an all-in-one, personalized support program that puts members at the center of their health care, and there’s no additional cost.

If you’ve received an invitation to join this program, you will work with a registered nurse who is part of your care management team. The Blue Cross Coordinated Care app helps you virtually connect with your nurse and access a variety of helpful resources.
