Health card App Online

Apply and download health card online. Also you can download and save pdf of Ayushman card for hospitals.

How To Get Ayushman Card:

1. Click on first option Ayushman Card.

2. Click on Continue if you have all the required documents.

3. Now click on ⭕ select option.

4. Select Scheme PMJAY.

5. Select your State.

6. Enter 12 Digit ID Number (xxxx xxxx xxxx)

7. Tick ✅ the checkbox box.

8. Click on Generate OTP

9. Enter OTP and click on Verify button

10. Click on Download Card button to save the PDF file.

How To Get Health card:

1. Click on second option Health card.

2. Select the option to apply new health card or download card of already registered user.

3. Enter 12 Digit ID Number (xxxx xxxx xxxx) for apply new health card.

4. Already registered user enter Mobile No. or ABHA No. for download health card.
