CHAmbassadors

Effectively provide feedback and opinions on our local NHS Health Services

The Community Health Ambassadors are a team of committed, friendly volunteers from the local

area who are very keen to hear your thoughts and experiences on local health services. Their role is

to act as the ‘eyes and ears’ of Hartlepool & Stockton-on- Tees residents on behalf of the Clinical

Commissioning Group and help influence the planning, development and delivery of the health

services that we use on a daily basis.

This app will help you to quickly and effectively provide feedback and opinions on our local NHS

Health Services including GP Practices, Hospitals, Pharmacies, Emergency Care and more. The

‘CHAmbassadors’ app is easy-to- use and allows us to share the results of any questions with you

immediately – alongside any follow up response or actions.
